Job Summary
Isolation to Connection assists isolated older adults by mobilizing the community resources and
necessary linkages to help meet their emotional, social, and physical needs. The Connection
Specialist listens to the needs expressed by the older adult, identifies meaningful services or
programs, navigates complex systems, advocates on behalf of the older adult to provide a
referral avenue that skillfully connects the older adult with necessary services and programs,
reducing isolation and loneliness. The part-time Connections Specialist will receive ongoing
clinical supervision from Westchester Jewish Community Services.
Key Responsibilities and Expectations:
● Part-time - 18 hours a week.
● Identify isolated older adults and successfully connect them with programs and services.
● Build strong relationships with the full continuum of older adult service providers,
government agencies, law enforcement, health care providers, not for profit agencies,
and faith-based organizations.
● Telephone contact with older adults, their family members, various elder care providers,
social service agencies, medical offices, as well as community and faith-based
organizations.
● Plan and present community outreach and education presentations.
● Travel to occasional community presentations needed. Access to transportation will be
necessary.
Skills, Education and Experience
● Social Service professional, MSW preferred.
● Familiar with Westchester County, New York’s varied services for older adults, social and
community services.
● Capable of working independently
● Self starter as this program is only in year 2 of the pilot
● Experience in telephone support and mental health triage services.
● Knowledgeable in work with older adults aging in community.
● Telephone based work may be done remotely if preferred.
● Excellent communication skills.
● Outstanding organizational skills as the program requires specific metrics and intake
processes.
● Excellent record keeping and reporting for grant purposes
Physical Requirements
The physical demands must be met by an employee to successfully perform the essential
functions of this job includes movement within an office environment. The role requires the
ability to write by hand and use a keyboard to perform general office functions. Also requires the
ability to communicate by speech and hearing continuously. Visual acuity is needed for detailed
work, and computer use. The ability to sit for extended periods of time is also a requirement.
