What You’ll Do:
• 🩺 Direct Care & Case Management: Provide direct and supportive health care to individuals in residential programs, assessing current health status and identifying needs.
• 🏥 Health & Safety Oversight: Provide professional advice and direction on health and safety issues, ensuring the ongoing support of individuals’ health and medical needs.
• 🤝 Collaborate with Treatment Team: Establish positive working relationships with individuals, families, and interdisciplinary team members, both within and outside the agency.
• ⚠️ Emergency Response: Initiate nursing interventions in emergency and non-emergency situations, adhering to established policies and clinical standards of care.
• 📝 Documentation & Care Plans: Establish and maintain nursing care plans, document individual health needs, and follow up on required actions.
• 📅 Appointment Scheduling: Ensure timely scheduling of medical appointments and services for individuals in your caseload.
• 👩‍⚕️ Staff Training: Provide training to staff on nursing care plans and agency in-services, ensuring proper care delivery.
• 💊 Medication Management: Oversee medication administration, ensuring proper labeling, storage, and disposal of expired medications according to policy and regulations.
• 🏥 Hospitalization & Discharge Coordination: Communicate with hospital discharge planners and ensure safe discharges for individuals who have been hospitalized.
• 📑 Health Record Monitoring: Track and update health records, including seizures, bowel movements, vital signs, and other health metrics.
• 💼 After-Hours On-Call: Be available for after-hour on-call duties, responding to situations in accordance with policies and regulations.
• 🛡️ Safety Compliance: Follow safety guidelines to ensure a safe working environment and report any hazards.
• 🌍 Program Coverage: May be required to float or cover other programs as needed or adjust work schedules to meet the needs of individuals.

What We’re Looking For:
• ✅ Education: Bachelor’s or Master’s degree in Nursing, with at least one year of professional experience in delivering health care to individuals with Intellectual/Developmental Disabilities.
• ✅ Licensing: NYS RN Certification is mandatory.
• ✅ Driver’s License: Valid unrestricted NYS Driver’s License and reliable transportation for travel between locations.
• ✅ Skills: Ability to triage urgent and crisis situations, problem-solve, and maintain focus in challenging situations.
• ✅ Flexibility: Must be flexible with days, hours, and location based on agency needs.
• ✅ Vaccination: Must be fully vaccinated (COVID-19).
