What Youβll Do:
β’ π©Ί Direct Care & Case Management: Provide direct and supportive health care to individuals in residential programs, assessing current health status and identifying needs.
β’ π₯ Health & Safety Oversight: Provide professional advice and direction on health and safety issues, ensuring the ongoing support of individualsβ health and medical needs.
β’ π€ Collaborate with Treatment Team: Establish positive working relationships with individuals, families, and interdisciplinary team members, both within and outside the agency.
β’ β οΈ Emergency Response: Initiate nursing interventions in emergency and non-emergency situations, adhering to established policies and clinical standards of care.
β’ π Documentation & Care Plans: Establish and maintain nursing care plans, document individual health needs, and follow up on required actions.

Appointment Scheduling: Ensure timely scheduling of medical appointments and services for individuals in your caseload.
β’ π©ββοΈ Staff Training: Provide training to staff on nursing care plans and agency in-services, ensuring proper care delivery.
β’ π Medication Management: Oversee medication administration, ensuring proper labeling, storage, and disposal of expired medications according to policy and regulations.
β’ π₯ Hospitalization & Discharge Coordination: Communicate with hospital discharge planners and ensure safe discharges for individuals who have been hospitalized.
β’ π Health Record Monitoring: Track and update health records, including seizures, bowel movements, vital signs, and other health metrics.
β’ πΌ After-Hours On-Call: Be available for after-hour on-call duties, responding to situations in accordance with policies and regulations.
β’ π‘οΈ Safety Compliance: Follow safety guidelines to ensure a safe working environment and report any hazards.
β’ π Program Coverage: May be required to float or cover other programs as needed or adjust work schedules to meet the needs of individuals.
What Weβre Looking For:
β’ β
Education: Bachelorβs or Masterβs degree in Nursing, with at least one year of professional experience in delivering health care to individuals with Intellectual/Developmental Disabilities.
β’ β
Licensing: NYS RN Certification is mandatory.
β’ β
Driverβs License: Valid unrestricted NYS Driverβs License and reliable transportation for travel between locations.

Skills: Ability to triage urgent and crisis situations, problem-solve, and maintain focus in challenging situations.
β’ β
Flexibility: Must be flexible with days, hours, and location based on agency needs.
β’ β
Vaccination: Must be fully vaccinated (COVID-19).
