What Youโ€™ll Do:
โ€ข ๐Ÿ“‹ Coordinate Personalized Plans: Develop and implement individualized plans for each client, including Staff Action Plans, Individual Plans of Protective Oversight (IPOP), Personal Expenditure Plans, Money Management Assessments, and Levels of Supervision.
โ€ข ๐Ÿค Lead Interdisciplinary Teams (IDT): Facilitate team meetings to ensure that clients receive quality active treatment and that their needs are met holistically.
โ€ข ๐Ÿ“… Monitor Progress: Track and evaluate the progress of clientsโ€™ goals and achievements, adjusting plans as necessary.
โ€ข ๐Ÿ“ Ensure Compliance: Maintain accurate, up-to-date electronic records and ensure all documentation is in compliance with OPWDD (Office for People With Developmental Disabilities) regulations.
โ€ข ๐Ÿ‘ฅ Support Independence: Assist clients in gaining independence with life skills, social interactions, and other personal goals.
โ€ข ๐Ÿ“š Provide Input for Training Programs: Help develop and implement training programs for staff and clients, monitoring progress and outcomes.

What Weโ€™re Looking For:
โ€ข โœ… Education: Bachelorโ€™s degree in a Human Services field (or related field) required.
โ€ข โœ… Experience: 1+ years of relevant experience in a similar role preferred.
โ€ข โœ… Skills: Proficiency in person-centered planning, instructional methods, and progress tracking. Strong organizational, communication, and interpersonal skills.
โ€ข โœ… License: Valid New York State driverโ€™s license is required.
โ€ข โœ… Knowledge: Understanding of OPWDD regulations and the ability to maintain accurate documentation.

Job Details:

๐Ÿ•’ Schedule: Mondayโ€“Friday (Full-Time, 40 hrs/week); flexibility required for occasional evenings and weekends.
๐Ÿš— Travel Required: Must be willing to travel between IAHD programs in Westchester and the Bronx.
