The Program Supervisor of the Critical Time Intervention (CTI) team will lead an interdisiplinary team of behavioral health professionals providing intensive, person-centered support to individuals with mental health and developmental disabilities as they transition from psychiatric settings into the community. This role ensures continuity of care, promotes recovery, and strengthens community integration through evidence-based CTI practices.

Position Overview:

The Program Supervisor will lead the CTI component of the Enhanced Step Down program, ensuring the CTI model is implemented with fidelity and participants receive high-quality, person-centered support. This role oversees care transitions for individuals leaving hospital or institutional settings and helps coordinate assessments, service linkages, and community integration efforts. Responsibilities include responding to referrals, supervising the CTI team members, aligning services with recovery goals, monitoring quality, tracking and compiling data, and collaborating with partner agencies to achieve successful outcomes.

Essential Responsibilities:

  • Oversee the day-to-day operations of the CTI component, managing care transitions and ensuring alignment with the phased CTI model.
  • Lead intake assessments and coordinate the development of individualized care plans, tailored to the specific needs of each client.
  • Ensure effective collaboration between hospitals, community providers, and CPU’s clinical teams, facilitating seamless transitions and service continuity.
  • Supervise the Care Manager and other direct service staff, providing guidance and support to ensure high-quality service delivery.
  • Monitor client outcomes, program fidelity, and adherence to performance metrics, making data-driven adjustments as needed.

Qualifications:

  • Education: LCSW, MSW, or LMHC required.
  • Master's degree in a related field may be consitered.
  • Experience: Minimum of five (5) years in care management and coordination within behavioral health and I/DD fields.
  • Skills: Expertise in care coordination, community integration, and recovery-focused interventions. Strong supervisory and communication skills.

Location: Multiple, including, but not limited to, CPU Hudson Valley Office, travel throughout the Hudson Valley, Transitional Residence, and community-based locations, including hospital referral sites
