 COMMUNITY TRANSITION COORDINATOR - Part Time (Non-Exempt)

Salary: $28.00 per hour.

 

Sign-in Bonus: A $2,000 sign-on bonus will be provided, with $1,000 paid after six (6) months of satisfactory employment and the remaining $1,000 paid upon successful completion of a one (1) year satisfactory employment review.

 

GENERAL FUNCTION: The Independent Living Specialist will be assisting consumers that are at risk of being placed in nursing facilities due to physical or cognitive disabilities or need to transition out of nursing homes, by helping them procure services and benefits needed to remain in their homes in the community or transition back into the community from institutions.

 

RESPONSIBILITIES:

  • Identifying services needed by consumers at risk of institutionalization to live in the
  • Prior to discharge, work with individuals to develop an Individualized Service Plan (ISP).
  • Establish periodic contact as required by WDOM and grantor to ensure participants have the needed service in place.
  • Provide independent living skills training, individual advocacy and other ongoing services as
  • Assisting with Wellness, Housing, Financial, and Social goals to promote
  • Implementing and guiding consumers and their caregivers through the service
  • Performing intake and documentation of consumers and their needs along with any required data entry in

the grantor’s dashboard and WDOM’s database.

  • Ensure deliverables are met as required by program funder and
  • Collaborate with applicant’s Discharge Planner, Care Manager and/or Service Coordinator and other

institution staff and WDOM staff to ensure services and supports are available to serve the individual

  • Communicating with consumers, caregivers, service providers, and other relevant
  • Assisting Consumers with applications for benefit
  • Advocating for adequate and timely provision of
  • Copying, scanning, faxing, and other clerical duties as
  • Other duties as
  • Outreach to Community Based Organizations, service providers, and healthcare providers to promote the program and Westchester Disabled On the Move.

 

JOB REQUIREMENTS & QUALIFICATIONS

  • Preferred Minimum of a bachelor’s degree in human services or related field, or Associate Degree with two

years of relevant experience working in the human services field.

  • Preferred candidates should have a working knowledge of issues faced by persons of all ages who need long-term care services, particularly those related to nursing home transition, and a deep commitment to person-centered planning and consumer choice.
  • Candidates should have basic knowledge of Medicaid eligibility and coverage,
  • Possess excellent written and oral communication and organizational skills
  • Ability to actively listen and, when appropriate, counsel throughout the transition
  • Must have time management skills to maintain outreaches, caseload, documentation, and administrative
  • Ability to work well independently as well as part of a
  • Computer literate on basic Microsoft Programs and Internet search
  • Respectful of people with a disability, from different cultural and socio-economic
  • Bilingual skills Fluency in a second language a plus.
  • MUST OWN TRANSPORTATION, HAVE VALID DRIVER'S LICENSE, INSURANCE AND THE ABILITY TO TRAVEL IN WESTCHESTER COUNTY.
